Session Spotlight: Improving Prescriber-Pharmacist ePrescribing Collaboration

Care models have moved away from individual provider care and are approaching patients with team-based care. Pharmacists are part of the care team, making message exchanges through tools, such as CancelRx and RxChange, particularly important to ensure harmonization and effectiveness of medications.

This session with Andrew Mellin, MD, MBA, Surescripts, and Samantha Pitts, MD, MPH, Johns Hopkins Medicine, will discuss new research on how standardized ePrescribing and different message types contribute to improved patient safety and coordinated care decisions and identify opportunities to better align the medication lifecycle with #prescribers and pharmacists.

Session Spotlight: Exploring Data-Driven Solutions to Improve Medication Outcomes

High performance on medication use quality measures is essential to reduce unnecessary healthcare utilization such as emergency room use, hospitalizations, and related adverse events. Medication-related quality measures also weigh heavily in the new Health Equity Index the Centers for Medicare and Medicaid Services (CMS) is implementing as part of the Five Star Rating System, including three triple-weighted medication adherence measures.

Tap into this session by Olufunke Pickering, M.D., MBA, Senior Medical Director, Inovalon, and Miranda Reich, POPM, Senior Product Manager, Inovalon, as they explore data-driven solutions to predict adherence and understand how patient demographics, comorbidities, medication history, and social determinants of health impact therapy adherence and treatment outcomes for diverse populations.

#NCPDP24 Session Spotlight: Digital Front Door: Driving Efficiency, Better Outcomes & New Revenue

Abandoned prescriptions and tedious manual workflows cost pharmacies money when margins are already tight. Led by Miranda Rochol, Vice President, Pharmacy & Provider Sales, Prescryptive Health, Inc., this accredited education session will describe how these technologies improve medication adherence, streamline workflows, and reduce administrative burden while freeing up capacity and capital to offer patient care services that drive new revenue.
