Boldly Defining the Pharmacy Patient Record

Pharmacy dispensing records are an incomplete record of the care provided in community pharmacies. The National Community Pharmacists Association convened a panel of experts and charged them with determining the essential elements and capabilities for a patient record in community pharmacy. This session will cover preliminary findings on the data and functional need pharmacists have as we seek to demonstrate quality measurement capability for value-based contracting.
