Use of Pharmacist eCare Plan/EHR Integration to Promote Team-based Care: Early Experiences in Bridging Community Pharmacists and Prescribers for Transitions of Care

A consistent barrier to including community pharmacists in team-based patient care has been inefficient and siloed communication between prescribers and pharmacists. There has been little advancement despite advances in healthcare technology use, including standards for exchanging electronic health records like Health Level Seven (HL7®) – serving as a barrier to collaboration and in sharing medical and pharmacy records. In 2021, a multi-stakeholder research team including the University of Tennessee and EnlivenHealth set out to demonstrate the value of integration of Pharmacist eCare Plan in the HL7® FHIR® format electronic health record integration. Several important barriers and facilitators have been uncovered. This session will describe the initial phase and lessons learned during the NCPDP Foundation-funded project building a bidirectional electronic communication link between a community pharmacy and medical office to provide transitions of care services.
