Exploring Data-Driven Solutions to Improve Medication Access, Adherence, and Outcomes

High performance on medication use quality measures is essential to reduce unnecessary healthcare utilization such as emergency room use, hospitalizations, and related adverse events. Medication related quality measures also weigh heavily in the new Health Equity Index the Centers for Medicare and Medicaid Services (CMS) is implementing as part of the Five Star Rating System, including three triple-weighted medication adherence measures. This session will explore data-driven solutions to predict adherence and understand how patient demographics, comorbidities, medication history, and social determinants of health impact therapy adherence and treatment outcomes for diverse populations.
